We are now using a completely online version of the form. Once filled out, it will automatically create a ticket with the information you provided. If you want to include an equipment request at the same time, you can add any additional information to the Notes field on the form.
You can access it through either the direct link or the business applications menu found in the Office 365 online portal. Note, that you will need to click 'Allow' when this pops up.

1. Start by clicking the waffle in the top left of your screen to access the apps menu
2. Select All apps
3. Select Show more
4. And finally, click the New User Request application
